I keep coming back to one number. Since January 2021, the reimbursement rate index for dental care has gone up 19%. Inflation over the same stretch was 27% (ADA Health Policy Institute, Q3 2026).
For the dentists I work with, that gap is more than a line on a chart. It's the hygienist raise you put off, the equipment upgrade that keeps slipping, and the extra day you work so the month closes.
I founded Rainey Dental Partners to build websites for dentists, so I'm biased. But here's my case: one of the most practical ways to rely less on insurance is also one of the least dramatic. Redesign your website so it attracts the cosmetic and high-value cases you want more of.
The insurance squeeze is real, and it's getting louder
Practices report contracted rates held flat for years, and in some cases cut on common procedures. Practices report harder preauthorizations, more denied claims and growing paperwork.
The research matches the mood. In the ADA HPI's Q3 2026 survey, dentists who felt skeptical about the dental sector named low reimbursement and insurance pressures as their top reason. And in April 2026, dentists filed class-action lawsuits against Delta Dental entities in four states, alleging conduct that suppressed reimbursement rates. Those are allegations, and the courts will decide them. Still, they show how many dentists feel about the current deal.
When most of your production depends on insurance, every rate change is a decision someone else makes for your practice.
Your competition is already diversifying
At the end of 2025, 35% of dentists said they were likely to drop some insurance networks in 2026. By Q3 2026, 25.3% said they already had (ADA HPI).
That matters for your marketing. Every practice that leaves a PPO network now needs the same patients you want: people who pay for quality, choose elective cosmetic work and say yes to a full treatment plan. In the same survey, 38% of dentists said they weren't busy enough and could have treated more patients. Chairs are open, and competition for the right patients is growing.
So diversify before your competition does. The practices that position themselves now are the ones patients find first.
What a redesign built for cosmetic and fee-for-service cases includes
When I say redesign, I don't mean a new color palette. I mean a site built around the patients you want more of:
- A custom site that tells your story. Template sites all look alike because they're the same website. A custom dental website shows patients why you're worth choosing.
- A page for every cosmetic service. Veneers, Invisalign, whitening and implants each need their own page to show up when local patients search for them. That's the core of how we approach cosmetic dentist websites and implant dentist websites.
- Membership and fee-for-service pages up front. Patients without insurance should be able to see, in plain language, how to work with you.
- Smile galleries where patients decide. Before-and-afters belong next to the service they prove. You can see how we handle this across our portfolio.
- Search built in. On-page and technical dental website SEO focused on the cosmetic and cash-pay services you want to grow, plus a migration plan so you don't lose your Google rankings when you switch.
The same thinking applies to specialists. A periodontist website should win implant and gum-grafting cases, and an orthodontist website should make adult aligner patients feel at home. For the basics every site needs, read what a dental website needs to convert new patients.
Why this is the small, practical step
Leaving a PPO network is a big decision with real risk. A redesign isn't. It pays off whether you stay in network and add a membership plan, go fee-for-service on some procedures, or leave networks entirely. It's the groundwork that makes every one of those paths easier.
It also keeps working. With dental website hosting, your site can change as your services, offers and plans change. If you're not sure your current site is holding you back, start with how to know it's time for a new dental website and the mistakes dentists make when buying a new website.
The patients you want are already paying their own way
You don't need to talk the whole market into paying out of pocket. A large share already does:
- 21% of adults ages 19–64 and 54% of seniors have no dental benefits, according to the ADA Health Policy Institute (the seniors figure doesn't account for Medicare Advantage plans).
- In 2025, 26% of U.S. adults, about 69 million people, said they had no dental coverage (CareQuest Institute).
- Traditional Medicare does not cover routine dental care, so many working adults lose their dental benefits when they retire (CDC).
Plenty of working adults without coverage can afford a membership plan and the treatment that follows. A clear in-house plan gives them a simple reason to book, and it gives you steady, predictable revenue that no payer can reprice.
But first they have to find you, and then they have to trust you.
Patients judge your care by your website before they call
This is why your website matters so much more than it used to:
- In Tebra's 2025 survey of nearly 4,000 patients, 44% said a practice's website influences their choice of provider, and 16% would consider switching if a provider's website is outdated.
- Press Ganey found that 59% of consumers use online search to find a new primary care provider, nearly as many as rely on referrals (60%).
- BrightLocal's 2026 survey found that after reading positive reviews, 54% of consumers go on to visit the business's website, and 27% have spent more than $1,000 after reading reviews.
- The same survey found that 45% of consumers now use ChatGPT or similar AI tools for local recommendations, which is why I wrote about how dentists are losing patients to AI search.
These numbers come from healthcare and local businesses in general, not dental alone, but the pattern is clear. Someone weighing a smile makeover or a full-arch implant case will research you before they ever pick up the phone. If your site looks like a template from 2016, you lose that patient without ever knowing they looked.
See your redesign before you decide
Here's my offer. Send us your practice info and your current website. We'll redesign your homepage around the cosmetic and fee-for-service patients you want, at no cost and with no obligation. You'll typically see the design within a few days, and we'll walk through it together. If you like it and approve, your new site goes live in 4–6 weeks.
You can read more about who we are or browse our FAQ. When you're ready, request your complimentary redesign, and let's get your practice in front of the patients who will pay for quality, before your competition gets there first.
